SHIITAKE DERMATITIS: A CASE REPORT AND REVIEW OF THE MEDICAL LITERATURE

Shiitake dermatitis is a cutaneous reaction caused by the ingestion of raw or undercooked Shiitake mushrooms. The physiopathology is not fully understood but is suggested to be due to lentinan, a thermolabile polysaccharide component of the cell walls of Shiitake mushroom. The skin eruption is characterized by erythematous linear papules, in a flagellate-like pattern, associated with intense pruritus and it usually involve the trunk and limbs. The skin lesion is self-limited, but the symptoms could be treated with antihistamine drugs and topical corticosteroids. A 28-year-old Caucasian male, with no pathological history, that
developed typical symptoms 72 hours after consumption of Shiitake mushrooms is presented. There is an increased ingestion of Shiitake in occidental countries and health professionals and dermatologists should, therefore, be aware of the characteristic skin lesions associated with its consumption.
