Se trata de una lesión en placa, exulcerada, con atipia basal notoria. Hay proyecciones de queratocitos atípicos hacia la dermis papilar, que permiten hacer el diagnóstico de queratosis actínica exulcerada, un sinónimo de carcinoma escamocelular muy superficial, incipiente o in situ.
