Henoch-Schönlein purpura in an adult patient: a case report

Henoch-Schönlein purpura is a systemic vasculitis of small blood vessels. It can affect any organ, mainly skin, joints, kidneys and the gastrointestinal tract.
Henoch-Schönlein purpura is the most common childhood vasculitis, even though adult cases have been described.
We described the case of a 36-year-old female who was diagnosed with type 2 diabetes; she presented with three weeks old itchy blistering lesions in her low extremities. Blood sugar test, lipid profile test and proteinuria tests all showed altered results. The histopathology study revealed an intraepidermal blistering and a leukocitoclastic vasculitis. Direct immunofluorescence showed a perivascular IgA deposit. We diagnosed a Henoch-Schönlein purpura case on an adult patient; therefore, we initiated an oral steroids treatment with 1 mg/kg of
patient for eight weeks. The patient has not presented new skin lesions or renal involvement after treatment though some scarring persisted.
We highlight importance of considering this disease in adults, even though Henoch-Schönlein purpura is an uncommon illness in this age bracket, patients always should be study for systemic involvement and related diseases.
